Insurance, Out-of-Network

At this time, I am an Oregon provider for Regence Blue Shield, MHN, Moda, and Pacific Source. Please note that even as a provider with a particular insurance company, each plan is different. Please call your insurance company and give them my name and confirm I am a provider for your plan. For all other insurance companies, I am considered an out-of-network provider. It is worth checking with your insurance to see if outpatient mental health benefits with an out-of-network provider is covered. Depending upon your plan, it may be covered and if it is, they usually cover 50-70% of their allowable. When you call your insurance company to see what the benefits are, be sure to find out if you need to have your annual deductible covered first for your benefits to apply.

If your insurance company does pay for out-of-network providers, you will pay my fee per session and I provide you what is called a "SuperBill", which is essentially a receipt with the CPT Codes and Diagnostic Codes the insurance company needs for reimbursement. You then submit the SuperBill along with the claim form to your insurance company for reimbursement to you. 

Insurance Questions

If you want to bill insurance for your sessions call your insurance company and ask the questions below. Bring this information to your first appointment.

1) For Out-of-Network: Ask, "Does my plan cover out-patient mental health benefits for out-of-network providers?

For In-Network: Ask, "What are my outpatient mental health benefits?

2) If yes, "What % is paid? or What is the co-pay?"

3)  "Does the deductible apply?" If yes, ask, "What is the deductible and how much has been paid to date?"

4) "Do I need authorization?" If so, obtain the authorization number from them right then and bring to the first appointment.

5) Are there limited number of sessions per calendar year or are the sessions unlimited due to medical necessity?


My fee is $100 with a sliding scale to $90 per 50 minute session. I accept Visa, MC, Discover, Check & Cash. If you need further consideration in regards to my fees for financial reasons, please contact me to discuss.

Local Office Parking

Parking can sometimes be a challenge to find on the street near my office. The parking behind my office building is for tenants only, however, the spaces are created to allow two cars to park, one behind the other. My parking space is #1, the space farthest to the left if you are facing the back of the building. You can park directly behind me in my space. It's important we conclude our sessions at 10 minutes to the hour so the next client will have a space behind me as well.

Waiting Room

Please enter the front of the building from 4th Street and make yourself comfortable in our waiting room to your left. I will come for you when it's time for our session to begin. If you are a new client, new client paperwork will be waiting for you with your name on it in the waiting room. For your first session, please arrive 10 minutes early to read, complete and sign the forms.
